Annual Physical vs. Executive Health Assessment: What’s the Difference?

By Dr. Michael Barber, Medical Director & Cardiologist

When it comes to protecting your long-term health, not all preventive care is created equal. While an annual physical exam is an important part of maintaining your health, an executive health assessment offers a far more comprehensive understanding of your current health status and future risks.

Understanding the differences between these two approaches can help you determine which level of care is right for your goals, lifestyle, and overall well-being.

What Is an Annual Physical?

An annual physical is a routine, insurance-covered wellness visit designed to monitor your general health. During the appointment, your primary care provider reviews your medical history, current medications, vaccinations, and any immediate health concerns. Standard laboratory testing and age-appropriate preventive screenings help identify common health conditions and monitor existing chronic diseases.

These appointments are valuable for maintaining continuity of care, but because they typically last between 15 and 45 minutes, there is often limited time to explore deeper health questions or identify emerging risks before symptoms appear.

What Is an Executive Health Assessment?

An executive health assessment is a much more in-depth, personalized evaluation designed to optimize health, enhance performance, and support longevity. Rather than simply identifying illness, an executive health program focuses on uncovering risk factors early and creating a proactive plan to improve health for years to come.

Every assessment begins with the fundamentals but expands well beyond a traditional physical. Depending on your health goals and needs, evaluations may include advanced diagnostic testing, comprehensive laboratory analysis, cardiovascular screening, body composition testing, functional movement assessments, nutrition counseling, stress resilience coaching, and other specialized evaluations.

These assessments typically take anywhere from a half-day to several days and provide a complete picture of your health that cannot be achieved during a standard annual exam.

Annual Physical vs. Executive Health Assessment

Annual Physical Exam

  • Primary Focus: Reviews current health concerns, medical history, medications, and preventive care.
  • Appointment Length: Typically 15–45 minutes.
  • Testing: Standard blood work, vital signs, BMI, and recommended age-based screenings such as mammograms and colonoscopies.
  • Cost: Often covered by health insurance (subject to deductibles or co-pays).
  • Provider: Usually performed by your primary care physician, who manages your ongoing healthcare.

Executive Health Assessment

  • Primary Focus: Preventive healthcare, early disease detection, longevity planning, peak performance, and stress management.
  • Appointment Length: Four hours to multiple days, depending on the scope of the evaluation.
  • Testing: Advanced diagnostics that may include cardiac imaging, EKGs, metabolic testing, body composition analysis, comprehensive laboratory studies, vision and hearing evaluations, nutrition assessments, and additional specialty testing.
  • Cost: Typically ranges from $2,500 to more than $15,000 and is generally paid out-of-pocket or offered as an executive benefit.
  • Provider Team: An integrated team of specialists—including physicians, cardiologists, nutritionists, chiropractic physicians, naturopathic physicians, and other health experts—working collaboratively to develop a personalized care plan.

The Strata Difference: Executive Health That Doesn’t End After One Day

Many executive wellness programs provide a one-day evaluation with minimal follow-up. While these assessments can identify important health concerns, meaningful improvements happen through ongoing guidance and accountability.

At Strata, we believe executive health should be a year-long partnership—not a single appointment.

Our Executive Wellness Program begins with a comprehensive, multi-day evaluation tailored to each individual’s schedule, health status, and personal goals. Throughout the assessment, patients meet with clinicians across multiple disciplines, each providing specialized insight into different aspects of health and performance.

Following these evaluations, our clinical team convenes for a multidisciplinary case review, collaborating to develop a fully integrated, personalized wellness strategy. We then meet with each patient to review the findings, prioritize recommendations, and establish a clear roadmap for the coming year.

This collaborative approach allows us to move beyond simply identifying health risks. We help patients actively improve their health through ongoing physician support, lifestyle optimization, performance coaching, and preventive care designed to maximize both healthspan and lifespan.

Investing in Long-Term Health

An annual physical remains an important part of preventive healthcare and should not be overlooked. However, for executives, business leaders, entrepreneurs, and high-performing professionals seeking a deeper understanding of their health, an executive health assessment provides a level of insight that a traditional physical simply cannot.

Your greatest asset isn’t your business—it’s your health. Investing in a comprehensive executive health program today can help you lead with greater energy, resilience, and confidence for years to come.
